Telehealth Informed Consent

By using Amie's telehealth services, you acknowledge and agree to the following:

1. Nature of Telehealth

Telehealth involves the delivery of healthcare services using electronic communications, information technology, or other means between a healthcare provider and a patient who are not in the same physical location. This may include assessment, diagnosis, consultation, treatment, and/or prescription of medications.

2. Consent to Telehealth Services

By completing your online health assessment and engaging with an Amie physician, you consent to receive healthcare services via telehealth. You understand that:

  • Your consultation will be conducted remotely through our secure platform
  • A board-certified physician will review your health information and determine if treatment is appropriate
  • Telehealth has potential benefits including improved access to care and convenience
  • Telehealth has potential risks, including the possibility that the provider may not be able to perform a physical exam

3. Medical Information

You agree to provide accurate, complete, and current health information. Providing false or misleading information may result in inappropriate treatment recommendations and puts your health at risk.

4. Privacy & Security

Your health information is protected under HIPAA (Health Insurance Portability and Accountability Act) and applicable state privacy laws. We use industry-standard encryption and security measures to protect your data. For more information, see our Privacy Policy.

5. Prescriptions

If your provider determines that prescription medication is appropriate, it will be prescribed and fulfilled through a licensed U.S. pharmacy. A prescription is not guaranteed — your provider may determine that treatment is not appropriate for you based on your medical history.

6. Emergency Situations

Telehealth is not appropriate for medical emergencies. If you experience a medical emergency, call 911 or go to your nearest emergency room immediately.

7. Right to Withdraw Consent

You may withdraw your consent to telehealth services at any time by contacting us at support@tryamie.com. Withdrawal of consent will not affect any services previously provided.

8. Follow-Up Care

Your Amie provider is available for ongoing follow-up care, dosage adjustments, and questions through your patient portal. You may also choose to follow up with your primary care provider at any time.

9. State-Specific Requirements

Telehealth services are available in states where our physicians are licensed. Service availability may vary by state. By using our services, you confirm that you are located in a state where we are authorized to provide care.
